The application must be lodged on the prescribed form. The application of claim has to provide information about name and surname or name of the firm, address and seat of the creditor and of the debtor, legal cause of the claim, ranking of the satisfaction of the claim from the debtor's estate and the amount of the principal and the interests with legal cause of the interests; the application of the claim has to be dated and signed by creditor. Documents proving the information provided in the application of claim have to be enclosed to the application of claim. Creditor who is the accounting entity in the application form include a statement indicating whether a claim is recorded in the accounts of the extent or the reasons why the claim is not recorded in the account books. 5. LEHOTY NA PRIHLASOVANIE POHLˇADA´VOK Pohlˇada´vky musia bytˇ prihla´sene´ najnesko^r do: 15.04.2024 Prihla´sˇka pohlˇada´vky sa poda´va v jednom rovnopise u spra´vcu, pricˇom spra´vcovi musi´ bytˇ dorucˇena´ v za´kladnej prihlasovacej lehote do 45 dni´ od vyhla´senia konkurzu. 5. TIME LIMITS FOR LODGING CLAIMS Claims shall be lodged at the latest: April 15, 2024 The claim has to be delivered to the office of Insolvency practitioner in one counterpart within the registration period of 45 days from the day of declaration of the bankruptcy procedure. 6. DO^SLEDKY PODANIA PRIHLA´SˇKY POHLˇADA´VOK PO UPLYNUTI´ LEHOTY UVEDENEJ V BODE 5

• Vylu´cˇenie z u´cˇasti na rozdeleni´ vy´tˇazˇku (predbezˇnom alebo konecˇnom), ku ktore´mu dosˇlo pred podani´m (alebo dorucˇeni´m) prihla´sˇky pohlˇada´vok.
• Strata hlasovacieho pra´va v akomkolˇvek rozhodovacom procese alebo na scho^dzi veritelˇov, a to pocˇas cele´ho konania.
• Na prednostne´ alebo zabezpecˇovacie pra´va spojene´ s pohlˇada´vkou prihla´sene´ po uplynuti´ lehoty podlˇa bodu 5 sa neprihliada a pohlˇada´vka sa povazˇuje za nezabezpecˇenu´.

Na prihla´sˇky, ktore´ nebudu´ spl´nˇatˇ za´konom stanovene´ podmienky alebo nebudu´ podpi´sane´ sa neprihliada. Na prihla´sˇky dorucˇene´ po uplynuti´ za´kladnej lehoty na prihlasovanie pohlˇada´vok sa prihliada, veritelˇ vsˇak nemo^zˇe vykona´vatˇ hlasovacie pra´vo. Na neprihla´sene´ zabezpecˇovacie pra´va sa v konkurze neprihliada. Spra´vca nie je povinny´ vyzvatˇ veritelˇa na doplnenie alebo opravu neu´plnej alebo nespra´vnej prihla´sˇky. Prihla´sˇku mozˇno opravitˇ alebo doplnitˇ len tak, zˇe sa po^vodna´ prihla´sˇka nahradi´ u spra´vcu novou prihla´sˇkou. 6. CONSEQUENCES OF FAILURE TO SUBMIT CLAIMS WITHIN THE TIME PERIOD INDICATED IN POINT 5

• You will be precluded from participating in distributions (interim or final) occurring before submission (or admission) of your claim.
• You will lose your right to vote in any decision-making process or at meetings of creditors during the whole proceeding.
• Your secured or preferential rights associated to the claim and lodged after expiry of period indicated in point 5 are not taken into account and the claim is considered as unsecured.

Any application of claim that will not fulfill the requirements stated by the law or will be not signed will not be considered as claims. The application received after the regular lodging period is taken into account, but the creditor cannot exercise the voting rights and other rights related to lodged claim. Not lodged security rights will not be considered. The insolvency practitioner does not have the duty to notify creditor to amend or to correct an incorrect or incomplete lodgment of claim. Application form may be corrected or amended only by replacing the original application form by a new application form. 7. DˇALSˇIE PODMIENKY PRIHLASOVANIA POHLˇADA´VOK Na za´klade pra´va uplatnitelˇne´ho na insolvencˇne´ konanie sa vyzˇaduje, aby sumy pohlˇada´vok (bod 6.1.8 sˇtandardne´ho formula´ra "Prihla´sˇka pohlˇada´vok") a na´klady spojene´ s uplatneni´m ty´chto pohlˇada´vok (bod 6.4.3 sˇtandardne´ho formula´ra "Prihla´sˇka pohlˇada´vok") boli uva´dzane´ v mene Euro (EUR). 7. OTHER CONDITIONS TO BE MET WHEN LODGING YOUR CLAIM The law applicable to the insolvency proceedings requires that the amount of the claim (point 6.1.8 of the standard form 'lodgement of claims´) and the costs arising from the assertion of that claim (point 6.4.3 of the standard form 'lodgement of claims´) are indicated in the currency Euro (EUR). 8. POVINNOSTI VERITELˇOV, KTORY´CH POHLˇADA´VKY SU´ PREDNOSTNE´ ALEBO ZABEZPECˇENE´ VECNY´M PRA´VOM

• v prihla´sˇke pohlˇada´vok musi´te vy´slovne uviestˇ osobitnu´ povahu pohlˇada´vok,
• pohlˇada´vky musi´te prihla´sitˇ v lehote podlˇa bodu 5,
• musi´te uviestˇ sumu, do vy´sˇky ktorej su´ pohlˇada´vky zabezpecˇene´.

Rovnaky´m spo^sobom ako je popi´sany´ v bode 4 tejto vy´zvy uplatnˇuju´ svoje pohlˇada´vky aj veritelia pohlˇada´vok, ktory´ch vznik je viazany´ na splnenie podmienky, pricˇom sa uvedie podmienka, od ktorej za´visi´ vznik pohlˇada´vky, a aj veritelia pohlˇada´vok zabezpecˇeny´ch vecny´m pra´vom, ktori´ v samostatnej prihla´sˇke uvedu´ druh, poradie, a pra´vny do^vod vzniku vecne´ho pra´va spolu s urcˇeni´m majetku, ktory´m je zabezpecˇena´ a sumu, do ktorej je zabezpecˇena´. Prihla´sitˇ sa mo^zˇe aj veritelˇ, ktory´ ma´ pohlˇada´vku vocˇi inej osobe ako dlzˇni´kovi, ak je zabezpecˇena´ zabezpecˇovaci´m pra´vom vztˇahuju´cim sa k majetku dlzˇni´ka. 8. OBLIGATION OF CREDITORS WHOSE CLAIMS ARE PREFERENTIAL OR SECURED IN REM

• you must expressly indicate the specific nature of the claim in the lodgement of claims,
• you need to lodge your claims in the time period indicated in point 5,
• you must indicate the amount up to which the claims are secured.

Creditors whose claims depend on meeting certain conditions lodge their claims in the same way as described in article 4 of this notice. The condition on which the claim is dependent has to be stated in the registration form. Creditors with claims secured with securities shall file individual application which includes specification of the secured amount of the claim, type, ranking, subject and legal reason of the arising of the security right. The creditor who has claim towards another person than the debtor, shall be entitled to file the application, if the claim is secured by the security right related to the property of debtor. 9.
